Level 4 Specialist Exercise Instructor Applications
Level 4 is awarded to individuals who can demonstrate that they have met the Level 4 National Occupational Standards in one or more medical areas, completing and passing one of the following approved Level 4 qualifications:
- BACR Phase IV Exercise Instructor Training
- WRIGHT Foundation Cardiac Rehab Phase IV
- Later Life Training Postural Stability Instructor (PSI)
- Active IQ Certificate in Exercise for the Management of Lower Back Back Pain
- Exercise after Stroke: Specialist Instructor Training Course
- Discovery Learning - Certificate in Obesity and Diabetes Management
- Exercise Rehabilitation Instructor (ERI) (Military Only)
Application is subject to completion of the L4 Confirmation of Pre-requisites and Professional Practice Unit Form which is split into two sections.
Section 1 details the pre-requisites for application and candidates must confirm that they have met each of the requirements. Pre-requisites may be downloaded here
Section 2 covers professional practice requirements and candidates must answer all questions correctly to be eligible for Level 4. Questions are multiple choice and based on the Professional Practice Paper which may be downloaded here

How To Register
Having completed the form and having answered all of the questions from the professional practice unit paper correctly you must print out the confirmation page and submit along with the following to be considered for Level 4.

PHOTOCOPIES of qualifications (do not send original copies). This should include copies of the required Advanced Fitness Instructor qualification, additional unit of qualification at Level 3 and Level 4 unit of qualification
Each photocopied certificate must be signed and dated by a 'responsible person' and submitted with a 'letter of authenticity', see below

Adequate Civil Liability Insurance - minimum cover £5 million ...can be purchased through REPs click here for details
Registration Fee and Insurance Fee (where purchased)...credit card or cheque (made payable to REPs)
